Panhandle Health District is seeking 
a full-time Licensed Practical Nurse (LPN)
to join our Home Health 
team
.


Responsibilities:
  • Performs practical nursing care and nursing interventions based on the orders of a physician and patient condition in the home setting.
  • Employs strategies to optimize patient encouragement.
  • Utilizes technology to measure, record and retrieve patient data, implement the nursing process and enhance nursing practice.
  • Assists patients in self-determination and informed decision making.
  • Validates the diagnoses or issues with the patient, family or other healthcare providers.
  • Documents diagnoses or issues in a manner that facilitates the determination of the expected outcomes and plan.

Minimum Requirement:

  • Licensed as a Licensed Practical Nurse by the state of Idaho or state licensure as a Practical Nurse pursuant to the Nurse Licensure Compact, Idaho Code 54-1418, in one of the following compact states: Arizona, Arkansas, Colorado, Delaware, Iowa, Kentucky, Maine, Maryland, Mississippi, Missouri, Nebraska, New Hampshire, New Mexico, North Carolina, North Dakota, Rhode Island, South Carolina, South Dakota, Tennessee, Texas, Utah, Virginia, or Wisconsin.

The ideal candidate will have excellent assessment and communication skills.  Prior home health experience is a plus.
