General Information:

Assist the Extended Employment Services Manager in case management on all active customers as well as those individuals on the waitlist. Provide case management to ensure all information provided is correct. That all information is present and analyze the information to determine what individualized support and funding is needed to obtain employment for the customer. Determines what outside vendor supports are needed for the customer to retain their employment goal.

Principal Accountabilities:

  • Reviews customer case records for appropriate vocational goals and documentation.
  • Provide case management services to all active and wait listed extended employment services participants.
  • Explains participant rights and responsibilities and provides information regarding the extended employment services program and processes.
  • Collects and analyzes information obtained from participant interviews, educational, medical records, consultation with other professional and diagnostic evaluations to assess participants’ abilities and needs to determine eligibility for program. 
  • Creates and provides various letters to customers such as eligibility, no funding, and verification.
  • Reviews and discusses Extended Employment Services mission with potential customers, guardians, vocational rehabilitation staff, etc.
  • Evaluates program activities to ensure efficient delivery of services.
  • Compiles and analyzes data to determine customer needs and ensure uniform application of policies and procedures.

Minimum Qualifications:

  • Two years’ experience working with individuals with disabilities. This experience must include collecting and analyzing information, service provision and assisting with and providing direct case management.

This position requires one year of work experience in each of the following:

  • Monitoring record systems to identify and correct errors
  • Interpreting, explaining, and applying laws, regulations, and complex policies for multiple or highly specialized programs
  • Analyzing information and researching a variety of sources to identify and resolve problems or issues
  • Dealing with individuals from varying cultural and socioeconomic backgrounds in stressful situations
  • Composing and proofreading business correspondence

You must also have six months of work experience:

  • Entering and retrieving data using a computerized record system 
Preferred Qualifications:
  • Excellent verbal, written, and interpersonal communication skills
  • Experience interviewing, gathering and analyzing data
  • In-depth experience in creating reports in Microsoft office Excel
  • Knowledge of measurable goals  to determine which course of action to pursue for customers
  • Analyzing information and researching a variety of sources to identify and resolve problems or issues
